REPORTS TO Health Center Manager (A0/A1)

JOB RESPONSIBILITIES

• detect hospitalized or outpatient social cases.

• Organize and classify social assistance cases.

• perform health education for patients and their families.

• Participate in health education sessions for other patients at the health center.

• Design micro-social assist services.

• assist and organize assistance for vulnerable, abandoned, or social cases.

• Visit patients at home to track their condition.

• Follow up on social worker care plans.

• Keep accurate records and produce reports.

• Ensure timely reporting, documentation, and archiving of all required reports.

• Perform any other duties assigned by the supervisor.

QUALIFICATIONS

• Advanced Diploma in Sociology

• A2 Certificate in Social Work

• Diploma (A1) in Social Work
